Please check your email to complete the new patient paperwork.  We appreciate you taking the time to fill out your Initial New Patient Paperwork to the best of your ability before your first visit.  Please Note: If your complaints are related to involvement in a motor vehicle accident or in a workman’s compensation accident, there will be additional paperwork to fill out prior to your first visit.

 
Please contact your insurance company to verify your coverage before scheduling an appointment.  Most HMO plans do not cover treatments performed at our office.
